Saint Francis Hospital Emergency Department
Under the general supervision of the Care Coordination Manager and as part of a multidisciplinary team, the
primary clinician's duties include, but are limited to, crisis intervention, psychosocial patient assessments and
interventions, mandated reporting, information and referral, non medical discharge planning , and coordination of health and mental health services with community provider. Works in concert with physicians, nurses, utilization management, discharge planners, and other allied health professionals. Assists patients with addressing and resolving the social, financial, and psychological problems related to their health condition and facilitates the patient movement through the health continuum.
This position focuses on removing barriers to care for high utilizers of emergency and acute care services, especially patients with high level of social determinants of health need. The primary clinician will need to have specialty in working with behavioral health conditions, substance use, and varying levels of health literacy, and be able to connect patients to housing navigation, enhanced case management, substance use resources, street medicine and other community supports.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $41.79 - $72.17 (Hourly Rate).

Required Qualifications
- Masters degree in social work, or related field from an accredited university.
- Demonstrated ability to provide case management in medical setting, including biopsychosocial assessment, mental status examination, comprehensive services navigation, and to provide crisis counseling, use motivational interviewing, develop rapport and advocate for patient's needs.
- Excellent documentation and clinical skills.
- Demonstrated ability to assess risk, including abuse/neglect/violence, suicide and grave disability.
- Demonstrated ability to work harmoniously with colleagues as a member of the treatment team in a fast paced, high pressure emergency room environment.
Preferred Qualifications
- Experience with dialectical behavior therapy and cognitive behavioral therapy groups.
- Experience with populations diverse in ethnicity, gender, and sexual orientation.
- Experience with acute psychiatric illness.
- Recent direct Patient care experience with DBT an/or Partial Hospitalization.
- Experience in hospital inpatient psychiatry or PHP/IOP
License/Certification
- For CSW2, registered associate with the Board of Behavioral Sciences in California.
- For CSW3, Licensed with the Board of Behavioral Sciences in the Sate of California (LCSW or LMFT)
