Community Nurse - Neurology

TCP Homecare provides innovative healthcare and pharmaceutical services to meet the requirements of a new emerging healthcare environment in Ireland. We are looking for innovative, enthusiastic nursing professional, who demonstrates a high degree of self-motivation, to lead a nationwide service within the home and community setting.


This is a nationwide, permanent, full-time role, Monday to Friday, with no weekends and Bank Holiday work required.


Job Scope:

To be directly accountable for the overall operational and clinical management of a Dedicated Neurology patient support programme.

The Dedicated Nurse Service Lead is responsible for providing and managing clinical services to patients in an effective and caring manner and to a high standard that meets with best practice while working closely with the multi-disciplinary team. The dedicated nurse will be responsible for all scheduling, co-ordinating and facilitating the entire patient support programme.


Duties and Responsibilities:


Service Delivery:


The provision of this Dedicated Nursing Service will include the following elements:

  • Assessment in the referring unit prior to initiating on treatment if requested by the referring unit.
  • Attendance in the Hospital to support the initial training
  • Post hospital Home Visit
  • On-going telephone support at pre-approved time intervals (to be decided)
  • Maintenance Visits with the Patient at Home at pre-approved time intervals (to be decided)
  • Provision of Reactive Visits
  • Staff training in hospital/respite facilities
  • Key contact for all communication from the multi-disciplinary teams
  • Key contact for the patient


Additional Responsibilities:


  • To act as dedicated service lead and to oversee those staff members participating on the service.
  • To closely co-ordinate, streamline and manage the service, maintaining at all times the highest clinical standards.
  • To lead referral acceptance processes and patient safety initiatives. To be the service ambassador in the hospital referral units providing a relevant support and educational platform for the multidisciplinary teams within the units.
  • To facilitate, lead and actively participate in liaison meetings between hospital referral units, the programme sponsor and TCP to maximise the value of the service and share key learnings.
  • To identify and deliver any proposed service enhancements as agreed from time to time with the client.
  • To meet the service Key Performance Indicators as agreed with the client.
  • To be responsible for the training schedule of all Nurses (new and ongoing) and work with the client on any training enhancements.
  • Education of clients/families and colleagues in relation to relevant disease areas.
  • Timely electronic reporting at each visit.
  • Maintenance of accurate and detailed records as agreed for each patient.
  • To liaise closely with all other healthcare personnel associated with services ensuring good lines of communication with the hospitals, consultants and other clinical personnel.
  • To liaise closely with the patient , if the patient requires additional visits
  • To prepare all patient schedules/ nurse visits in a timely manner
  • Assess specific changes in the patient's condition as a result of new or on-going treatments and take immediate appropriate action.
  • Recognise and report immediately any untoward events on a patient visit. Report all adverse events and product complaints promptly in line with Pharmacovigilance procedures.
  • Implement and update all TCP Homecare operating procedures and protocols relating to the care and safety of a patient and yourself.
  • Attend relevant meetings with company and/or clients to provide oversight and details of the service and facilitate meticulous feedback.
  • Participate in approved data collection, clinical studies and trials as requested by the client.
  • Keep abreast of current trends and developments in professional matters and in clinical care. Ensure that clinical skills are maintained.
  • Responsible for the administration of daily/weekly/monthly documentation for departmental reviews.


Requirements:


Qualified RGN- NMBI registered

Neurology background highly desirable

Applicant must show competence in leading nursing teams/services

Must possess a strong work ethic while demonstrating ability to work under own initiative

Must have the ability to educate, communicate and present while building a rapport with a range of key stakeholders

Must have a systematic and structured approach to managing client services

Must be flexible and available to travel nationwide with some overnight stays required

Must have a full clean driving licence

Must be competent in Microsoft excel, outlook & PowerPoint
