Experienced Support Worker for High-Level Physical Support needed
We are seeking an experienced Support Worker to provide high-level physical support and care for a participant who is paralyzed and requires comprehensive assistance with daily living, medication administration, and catheter management. The ideal candidate will have significant experience in a similar role and be committed to enhancing the participant's quality of life.

Key Responsibilities
Mobility and Transfers:
- Assist with rolling from side to side using a hospital bed with rails.
- Support two-person bed transfers using appropriate lifting equipment.
- Assist with transfers from bed to wheelchair/commode.
- Ensure safety during transfers and mobility activities.
Personal Care:
- Assist with toileting and continence management, including pad changes and catheterization.
- Provide regular skin care to prevent pressure injuries, including repositioning every 2-4 hours.
- Assist with full showering and sponge washing.
- Help with dressing in bed, considering limited limb mobility and shoulder pain.
Medication and Health Management:
- Set up and administer medications, ensuring the participant understands instructions.
- Administer bowel management as required.
- Monitor and manage pain levels, ensuring timely administration of pain relief medications.
- Work closely with nurses and healthcare professionals to manage catheter care and other medical needs.
Daily Living Support:
- Prepare meals and assist with feeding as necessary.
- Perform light domestic tasks such as cleaning, laundry, and home maintenance.
- Support in managing daily routines and schedules.
Community Engagement:
- Facilitate participation in community activities, including medical appointments and leisure activities.
- Assist with transportation using a powered wheelchair and maxi taxi.
Behaviour Management:
- Manage and de-escalate aggressive behaviors triggered by unmet needs or delays.
- Follow behaviour support strategies to ensure the participant feels heard and respected.
- Maintain clear and respectful communication, setting realistic timeframes for tasks.
Overnight Support:
- Provide active overnight assistance for pad changes, pressure area care, and transfers.
- Respond promptly to calls for assistance during the night.
Qualifications:
- Extensive previous experience in a similar role providing high-level physical support.
- Knowledge of using mobility aids, including hoists and powered wheelchairs.
- Understanding of pressure care management and continence care.
- Excellent communication skills and the ability to manage challenging behaviors.
- Current First Aid and CPR certification.
- National Police Clearance and Working with Children Check.

- Strong physical stamina and the ability to perform manual handling tasks.
- Compassionate, patient, and able to build rapport with participants.
- Ability to work independently and as part of a team.
- Problem-solving skills and adaptability in dynamic situations.
- High attention to detail and organizational skills.
Working Conditions:
- Work involves physical tasks such as lifting, bending, and transferring.
- Shifts may include early mornings, evenings, and overnight support.

- Certificate IV in disability (Required)
- NDIS Worker Orientation Induction Modules (Required)
- First Aid & CPR (Required)
- NDIS Worker Screen check (Required)
- Australian National Police Check (Required)
- Full driver's license (Required)
- Working with Children Check - Employer (Required)
- Infection Control Training (Required)
- Fully vaccinated (three doses which includes the booster dose) – mandatory

- Previous experience supporting people with a disability - Mental Health
- A reliable vehicle with full comprehensive car insurance.
Qualifications & Experiences
- Support participant to participate in age-appropriate activities and tasks including employment, in home support and community access.
- Support clients in accessing activities and outings in the community.
- Provide direct assistance to each client in accordance with their service plan and as directed.
- Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure the completion of appropriate documentation. Relevant information on support provided should be recorded in client notes and communication books as required.
- Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
- Work with clients, their families/ carers, relevant case managers, and other professionals to support the clients' individual goals.
- In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
- Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
- Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
- Liaise extensively with other staff to ensure the service's overall smooth and consistent running.
- Report regularly to the manager
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
- Provide backup and support to other staff, as necessary.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Notify the service if you are unable to work your rostered shifts with as much notice as possible.
- Contribute to Case reviews with Employment Consultants and Facilitators.
- Take reasonable care for his or her own health and safety
- Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons.
- Report any work quality and safety incidents or hazards to their Manager or Supervisor.
- Carry out their roles and responsibilities as detailed in the relevant policies and processes.
- Keep themself fully informed of the contents of the NDIS system and apply it to daily work
