Family Nurse Practitioner-Weight Loss Clinic-Baseline 635

Nurse Practitioner (NP) will provide comprehensive care for adults with obesity, diabetes, and related metabolic conditions, working closely with a clinical pharmacist, dietitian, and behavioral health specialist.

Essential Functions:

• Conduct comprehensive assessments, develop individualized treatment plans, and monitor progress for patients with obesity and diabetes.
• Manage GLP-1 and other anti-obesity/diabetes medications in collaboration with the clinical pharmacist, ensuring safe initiation, titration, and monitoring.
• Provide education and counseling on nutrition, lifestyle changes, and self-management strategies in partnership with the dietitian and behaviorist.
• Monitor clinical outcomes, address side effects, and adjust treatment plans based on patient response and evidence-based guidelines.
• Participate in team huddles and interdisciplinary case reviews to ensure coordinated, high-quality, patient-centered care.
• Maintain accurate, timely clinical documentation and contribute to quality improvement initiatives within the clinic.
• Maintains regular and predictable attendance.
• Performs other duties as assigned.

Position Qualifications:

Minimum Qualifications:

• Approved Nurse Practitioner education program, experience in the clinical area of specialty.
• Valid Arizona R.N. License.
• Valid Arizona Nurse Practitioner certificate.
• Valid CPR certificate.

Preferred Qualifications:

• 1 year of experience in Family Medicine or Geriatrics.
• Strong interest in obesity medicine, diabetes care, and chronic disease management.
