Job Description:

We are seeking a dedicated and compassionate Home Health RN Case Manager with at least one year of experience to join our growing team. In this role, you will be responsible for managing the overall care of our home health patients, ensuring they receive the highest quality of care in the comfort of their homes.

Key Responsibilities:

• Conduct comprehensive assessments of patients' health conditions and develop personalized care plans.
• Coordinate with physicians, caregivers, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Monitor patients' progress and adjust care plans as necessary to achieve optimal outcomes.
• Educate patients and their families on health conditions, medications, and self-care techniques.
• Maintain accurate and timely documentation of patient care, including assessments, care plans, and progress notes.
• Advocate for patients' needs and ensure they receive appropriate services and support.
• Ensure compliance with all state and federal regulations, as well as company policies and procedures.

Qualifications:

• Active RN license in the state of TX.
• Minimum of 1 year of experience as a Registered Nurse, preferably in a home health or case management setting.
• Strong clinical skills and the ability to make critical decisions regarding patient care.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Proficient in using electronic health records (EHR) and other healthcare software. Axxess Preferred.

Pay: $81,002.00 - $82,435.00 per year
