Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) for our Home Health division. In this role, you will provide high-quality patient care in the comfort of patients' homes, ensuring their health needs are met through skilled nursing services. The ideal candidate will have a strong background in various nursing specialties and be committed to delivering exceptional patient service.

Responsibilities

• Conduct comprehensive patient assessments to develop individualized care plans.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Monitor vital signs and evaluate patients' conditions, documenting changes accurately in medical records.
• Collaborate with interdisciplinary teams to coordinate care and discharge planning for patients.
• Provide education and support to patients and their families regarding health management and disease prevention.
• Utilize critical thinking skills to address complex patient needs, including those with acute and chronic conditions.
• Maintain compliance with HIPAA regulations and uphold the highest standards of patient confidentiality.
• Participate in case management and utilization management activities to optimize patient outcomes.

Qualifications

• Current Registered Nurse (RN) license in the state of Texas.
• Familiarity with geriatrics, infectious disease care, and medical-surgical nursing is preferred.
• Strong knowledge of medical terminology, physiology, and discharge planning processes.
• Excellent patient assessment skills with a focus on delivering high-quality patient care.
• Ability to work independently while managing multiple priorities effectively.
• Strong communication skills for effective collaboration with healthcare teams and patient education.

Pay: $75,000.00 - $90,000.00 per year
