Home Health Rn Case Manager - New Grads Welcome - Part Time/Per Diem

We are a faith-based community and we are looking for nurses who are dedicated to providing the best care for patients and wants to grow with our incredible team! We are looking for per diem RNs. Potential opportunity to transition to full time for interested candidates.

RN Case Manager Position Summary

• Performs patient assessments based on agency standards of practice to ensures effective and appropriate home health care services.
• Directs, plans and initiates appropriate action independently and responsively in home health care situations.
• Supervises LVN's, home health aides and personal care aides to provide effective and quality home health care services.
• Completes assigned tasks within established guidelines and time frames.
• Documents OASIS assessments accurately in the system within the given time frames.

Qualifications:

• Current California RN License
• Friendly/ Excellent customer service and great bed side manners
• Good Listener
• Wound Care experience or certification
• Minimum of one year experience as an RN

Pay varies based upon home health experience and documentation timeliness.
