Kidney Dialysis Centre – Senior Staff Nurse

Job Function

  • Responsible to support the Dialysis Centre to deliver holistic care by assisting in all aspects of the management to achieve maximum efficiency and cost-effectiveness in the facility while maintaining a safe environment that is supportive of optimal patient care.

Roles and Responsibilities

Administrative Responsibilities

  • Record and update all the patient’s relevant medical history and related data and ensure to maintain confidentiality of information
  • Participate in cost control initiatives
  • Always maintain the facility in a clean, safe and sanitary environment
  • Involve in stocks inventory
  • Assist the Nurse Manager in doing the staff and patients’ roster
  • Assume other additional duties and responsibilities assigned by Nurse Manager

Staff Related Responsibilities

  • Ensure all equipment and consumables meet standard requirement, at optimal working condition safe for patient’s use and with enough available stocks to use.
  • Ensure timely reporting of incidents (relating to clinical incidents, technical issue, patients complain, compliance issue etc.)
  • Attend morning (AM) to afternoon (PM) staff meetings for proper handovers.
  • Lead the team for any quality improvement projects as assigned by the Nurse Manager
  • Assist to train the cleaner

Clinical Duties

  • Ensure care delivery to patients in accordance to Key Performance Indicators.
  • Demonstrate competency in dialysis-related procedures including the care and procedures of dialysis access such as cannulation of arterio-venous fistula (AVF) and arterio-venous graft (AVG) and preparation of central venous catheter (CVC) based on prescription and according to the expected standards of practice and guidelines.
  • Preparation, commencement and completion of dialysis treatment, management of dialysis complications with support from Staff Nurse, responsible to clear dialyzers and bloodlines after dialysis, etc.
  • Responsible in assessing and documenting patient’s parameters and psycho-physio-social needs before, during and after treatment. Report any abnormalities to Staff Nurse timely.
  • Demonstrate competency in collection of patient blood sample
  • Demonstrate competency in dialysis equipment related procedures according to protocol including troubleshooting, disinfection, care and maintenance.
  • Practice primary nursing and treatment plan for assigned patients in collaboration with Staff Nurse.
  • Assist the Staff Nurse during doctor consultation round
  • Ensure patients and their families are educated on renal dialysis care by regular communications together with the Staff Nurse
  • Ensure accurate, complete and timely documentation of patients’ clinical notes which includes care provided and medications administered.
  • Provide update blood test results on a timely basis to ensure accurate and timely reporting of RenalTeam’s Clinical Outcome and Quality Measures.
  • Assist the Staff Nurse in collection of water and dialysate samples to send for quality tests.
  • Participate in quality improvement projects to pursue quality outcomes for patients.
  • Adhere to proper aseptic techniques and infection prevention and control protocol.
  • Coordinate with external care provider to ensure patient receive continuous care e.g., getting medical appointment, assist to order medications on online platform, referring to social worker, etc.

Other Responsibilities

  • Maintain a safe and clean environment for the patients at all times.
  • Ensure services delivered are prompt, professional and customer-focused at all times.
  • Effectively handle customers’ inquiries, complaints, and feedback to achieve positive resolution.
  • Facilitate evaluation and continuous improvement of services.
  • Attend continuing education to upgrade nursing knowledge
  • Perform any other tasks as assigned by the Nurse Manager and HQ.

Requirements

  • NITEC in Nursing
  • Valid registration with the Singapore Nursing Board
  • Renal nursing experience is preferred but applicants without renal nursing experience are also encouraged to apply.
  • Valid certificate of Basic Care Life Support (BCLS)
  • Good verbal and written communication skills
  • Excellent customer service and customer relation skills
  • Able to work independently as well as in a team
  • Effectively bilingual in two languages (English and Mandarin/Malay/Tamil with one or more dialects) is preferred
  • Good knowledge of computer application

Work Experience

  • Minimum 2 years of relevant working experience in a healthcare environment, including renal nursing experience.
