BACKGROUND:
Ontario Health Teams are tasked with the mandate to redesign health care delivery to provide team-based, integrated care that is more accessible, timely, and effective for patients. With its care providers’ deep roots in the community, the NT OHT is committed to developing seamless access to team-based, integrated care in a timely way through one connected system of providers who take a holistic approach to patient care with the patient at the center to improve health care experiences and outcomes for those we serve. Sunnybrook, on behalf of the NT OHT, is seeking a Project Assistant to support the immediate goal of attaching patients to primary care. The incumbent will be part of a bigger team, supporting the administrative activities required to facilitate patient attachment to primary care. This position will work collaboratively with numerous internal and external stakeholders, including local primary care providers and OHT members. This role will report to Director, North Toronto Ontario Health Team at Sunnybrook.
Summary of Duties
- Works closely with the broader project team to facilitate patient attachment to primary care. Activities include but are not limited to:
- Support the identification of individuals who are unattached to primary care
- Contact unattached individuals to understand their needs in facilitating connection with primary care
- Assist in the development of information and communication materials related to the project of facilitating attachment
- Support data entry, reporting and information sharing across key stakeholders, including our funders
- Contribute to the development and maintenance of relevant stakeholder databases
- Assist in the monitoring and tracking of project progress, and provide written or verbal updates as required
- Participate in respectful active engagement and relationship building with patients, community members and primary care providers
- Collaborate with OHT project leads and share relevant feedback to support primary care attachment
- Perform other routine and regular day-to-day activities as assigned
Qualifications/Skills:
- Minimal 2 years’ of work experience required; experience in healthcare an asset
- Baccalaureate Degree is preferred.
- Demonstrated experience working with people, and knowledge of engagement is required.
- Experience working with digital tools is required (i.e. databases, graphic design tools)
- Must have excellent working knowledge of Microsoft Office (Outlook, Word, Excel, PowerPoint)
- Ability to work effectively in highly demanding environment.
- Excellent interpersonal, verbal and written communications skills are required
- Excellent ability to prioritize and organize work and consistently demonstrate initiative to improve processes.
- Flexible thinker, with an ability to quickly adapt to a changing environment.
- Excellent written and oral communication skills to support our requirement to read, edit and compose correspondence
- Must be able to demonstrate flexibility as well as a comfort level with taking initiative on a wide range of issues
- Good problem-solving skills and an ability to work as a team member on projects
- Excellent analytic skills and judgment with ability to research and summarize administrative data
- Excellent attendance
