What You'll Do

You will serve as the nutrition and diabetes education expert on an interdisciplinary care team alongside primary care providers, nurses, community health workers, and behavioral health specialists. Your day-to-day will include:

  • Medical Nutrition Therapy (MNT): Provide individualized, evidence-based MNT for patients with type 2 diabetes and prediabetes. Develop culturally responsive meal plans that meet patients where they are — including those navigating food insecurity.

  • Diabetes Self-Management Education & Support (DSMES): Deliver individual and group DSMES sessions covering the ADCES7 Self-Care Behaviors — healthy eating, being active, monitoring, taking medication, problem solving, reducing risks, and healthy coping. Help build and sustain an ADA-recognized or ADCES-accredited DSMES program.

  • CGM & Technology Support: Educate patients on continuous glucose monitoring (CGM) devices, help interpret glucose data, and collaborate with providers to optimize therapy based on CGM trends and patterns.

  • Individualized Meal Planning: Create practical, personalized meal plans that honor cultural food traditions, accommodate limited budgets, and empower patients to make sustainable changes.

  • Team-Based Care: Partner closely with community health workers and peer educators to reinforce nutrition goals, address social determinants of health, and ensure seamless follow-up. Serve as a clinical resource for the broader care team on nutrition and diabetes management.

  • Program Development: Contribute to the design, implementation, and continuous improvement of our Chronic Disease Management Program. Help develop curricula, patient education materials, and outcome tracking systems.

Strongly Preferred:

  • Certified Diabetes Care and Education Specialist (CDCES) credential — or actively pursuing certification within 12 months of hire
  • Bilingual in English and Spanish (spoken and written) — our patients deserve care in the language they think, dream, and cook in
  • Experience with continuous glucose monitoring (CGM) technology and insulin therapy education
  • Experience working in an FQHC, community health center, or safety-net setting
  • Familiarity with ADA Standards of Care, ADCES accreditation/recognition processes, and MNT/DSMT billing
  • Experience developing or facilitating group education classes

Requirements:

  • Bachelor's degree (minimum) in Dietetics, Nutrition, or related field from an ACEND-accredited program
  • Current Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR)
  • Active state licensure/certification as required by South Carolina LLR
  • Minimum of 2 years of clinical experience, preferably in diabetes care, community health, or outpatient settings
  • Current Basic Life Support (BLS) certification
