Position Overview
We are currently seeking an experienced and patient-focused Medical Assistant to support our Remote Patient Monitoring (RPM) and chronic care coordination services for geriatric patients. This fully remote role is ideal for someone who is confident communicating with patients over the phone, passionate about patient education, and capable of helping seniors better understand and manage their health conditions.
The ideal candidate is dependable, organized, compassionate, and comfortable working in a fast-paced remote healthcare environment.
Essential Responsibilities
- Support Remote Patient Monitoring (RPM) and chronic care management programs for geriatric patients.
- Educate patients on chronic conditions such as diabetes, COPD, hypertension, and heart disease.
- Guide patients regarding medications, preventive care, monitoring compliance, and healthy lifestyle habits.
- Communicate with patients, caregivers, providers, and healthcare teams to coordinate ongoing care needs.
- Document patient interactions and updates accurately within the Electronic Health Record (EHR/EMR) system.
- Assist with medication refill coordination and routine follow-up calls.
- Collaborate closely with clinical staff to support individualized patient care plans.
- Perform scheduling, phone support, and additional administrative duties as assigned.
Qualifications
- High school diploma or equivalent required.
- CMA or RMA certification preferred.
- Minimum 1–2 years of healthcare experience preferred, especially in geriatrics, RPM, chronic care management, or care coordination.
- Prior remote Medical Assistant experience preferred.
- Strong understanding of chronic conditions commonly affecting senior populations.
- Experience using EHR/EMR systems required.
- Excellent communication skills with the ability to confidently educate and engage patients.
- Detail-oriented, organized, and able to multitask effectively.
- Compassionate and patient-centered mindset.
Schedule & Work Environment
- Monday through Friday
- 8:00 AM – 5:00 PM with lunch break
- Fully remote position with heavy telephonic patient interaction
