On An Average Day You Will:
- Create and initiate care plans through assessing the history of current and previous illnesses
- Spend quality one-on-one time with patients and their families to support everyday needs
- Care for patients by providing initial and ongoing assessments of patient and family needs
- Record important health information including temperature, pulse, respiration, and blood pressure
- Administer medications and treatments as prescribed by the physician
- Provide health care instructions to the patient as appropriate
- Record symptom management changes and outcomes
- Complete, maintain, and submit accurate and relevant clinical notes regarding the patients condition and care given
- And additional duties as assigned
Qualifications:
- An enthusiasm for providing the best possible care to patients and their families
- An RNlicense in good standing
- 1 year of experience in hospice or home health preferred
- A valid drivers license
- Current CPR certification
- Excellent observation, verbal, and written communication skills
- And additional related duties as needed
