Job Description

You will be part of the Community Case Management Service (CCMS) team, providing support to vulnerable seniors and their caregivers who face difficulties in managing their multiple health and social care needs.

Key responsibilities:

  • Conduct intake and triage services for members of the community
  • Provide casework, counselling and service coordination for seniors and caregivers
  • Conduct psychosocial assessment and coordinate interventions with healthcare and community professionals to support seniors and caregivers
  • Document and review case, interventions and outcomes within set timelines
  • Ensure case documentations are submitted in a timely manner
  • Liaise and work with organizations, partners and volunteers to build an ecosystem of assets and resources to facilitate more effective work with seniors and caregivers.
  • Assist with report submissions or other projects, as assigned by Service Head/Manager
  • Opportunities to be involved in organizational level projects.

Qualifications

  • Degree/Graduate Diploma in Social Work
  • At least 2-3 years of relevant working experience with good knowledge in eldercare or healthcare setting would be advantageous.

Requirements:

  • Passionate to work with seniors and caregivers
  • High proficiency in written and spoken English; Able to speak in mandarin and/or dialects would be essential for engagement with seniors
  • Strong interpersonal and communication abilities with individuals and systems at multi-levels
  • Good analytical and assessment skills
  • Team player with an ability to function effectively in a fast-paced environment
  • Strong organizational skills and structured.
