What You’ll Do:

In this dynamic and rewarding role, you’ll:

  • Partner with participants to design and implement personalised, strengths-based NDIS plans.
  • Connect people with the services and supports that align with their goals and needs.
  • Use recovery-oriented and person-centred approaches to build skills and confidence.
  • Coordinate service delivery, monitor progress, and adapt plans as required.
  • Collaborate with a multi-disciplinary team and contribute to service innovation.
  • Maintain accurate records and ensure alignment with NDIS practice standards.

What You’ll Bring:

  • Significant experience in the disability or human services sector.
  • Knowledge of intervention models and ability to coordinate services.
  • Specialist knowledge of NDIS and related support services.
  • Strong problem-solving and conflict-resolution skills.
  • Ability to advocate effectively for participants and promote their independence.

Your Toolkit:

  • Tertiary qualification in community services, social work or equivalent and extensive community work and training (essential)
  • Current DHS Working with Children Check (essential)
  • Current DHS NDIS Worker Screening Check (essential)
  • Current SA driver’s licence (essential)
  • Current Safe Environments for Children and Young People – Through Their Eyes Certificate (essential)
  • NDIS Orientation Module (essential)
