Description:
Position Overview

Lead the transfer of community care services from Yarra City Council to YourCH. Shape inclusive, client-focused support by collaborating with partners, applying co-design, managing risks and improving service delivery across the community care network. Reporting to the Community Programs Manager, this is a fixed-term, part-time role (0.5 FTE) concluding Friday 30 January 2026.

• Lead HACC-PYP service shift into YourCH’s care model
• Work with teams and partners to shape service delivery
• Represent YourCH at key forums and networks
• Run working groups to drive system change
• Co-design improvements with clients and community

To succeed in this role you will have:

• Qualification in health, disability, aged care or related field
• 5+ years' experience in health, aged care or community services
• Proven project leadership and project management skills
• Skilled in building and strengthening partnerships
• Experience using co-design in planning and service delivery
